Position Purpose Assists residential clients with care, supervision, support, and daily living skills for individuals who have multiple disabilities. Responsible for scheduling and transporting clients to and from health care appointments. Orders supplies, and maintain inventories. Works with licensed health care professionals and clients; coordinates medication requirements of residential clients. In the absence of the Site Program Manager, occasionally oversees daily program site operations when the Program Manager is absent. These responsibilities will include overseeing that all daily routines of residence are followed and that all resident/staff coverage occurs at the residential program site. Position may from time to time be stressful and require a high demand of performance. May perform other duties as assigned. Essential Functions Client Support Schedules and transports clients to and from heath care visits Communicates results of health care visits to team members Ensures follow up recommendations are implemented Works with licensed health care professionals, clients and or guardians; coordinates the medication requirements of residential clients, including set up of medisets and preparation of medication records monthly Assists with the preparation and presentation of Individual Support Plans (ISP) and Medication Reviews Assists with the planning for age appropriate group and individual outings Complies with all licensing documentation requirements, progress, and outcomes of clients Assists residents on a daily basis with personal care such as hygiene or first aide, either as directed by client, care plan, or Individual Support Plan (ISP) Assists residents with transferring, lifting, and moving personal items Assesses environment for residents using adaptive equipment Implements and follows Individual Support Plan (ISP) Administration Communicates with health care professionals, guardians, and program staff to ensure all licensing documentation requirements are completed according to policy Provides job related training to staff, as directed Complies with all licensing documentation requirements Assists in providing and scheduling appropriate staffing coverage Assists with maintaining accurate and complete client files, progress reports, and medication reports required by policy or licensing Assists with site compliance with DES, DDD, and DHS licensing regulations, as well as all Corporate, Agency, and Program policies and procedures Ensures required documentation of client funds according to regulatory requirements, and Corporate, Agency, and Program policies and procedures Responsible for maintaining appropriate levels of food and supplies, and inventories of residents' homes Attends all required resident, staff, and in-service training and/or meetings Residential Site Operation Ensures site equipment is properly maintained Coordinates monthly fire drill evacuations and documentation Maintains living environment in a clean, sanitary, and safe condition, and completes home safety checklist, as directed Creates an atmosphere of home and engages members in recreational and social activities Creates a weekly plan with residents for recreational and social activities Responsible to ensure that nightly bedtime routines for the residents are followed Follows cleaning schedule Completes weekly inspection forms for apartments or home Completes van inspection logs Weekend duties Ensures member/residents have breakfast and lunch as needed for day program Ensures that transportation is coordinated and provided to member/residents Responsible that daily check list of cleaning duties is completed Responsible that members' files are up to date with planning documents, BTP, and information sheets, etc. Creates an atmosphere of home and engages members in recreational and social activities Creates a weekend activity schedule with residents including recreational and social activities Follows cleaning schedule Performs fire drills and checks first aid kits Performs health and safety inspections Orders supplies MINIMUM QUALIFICATIONS Suitable work experience may be considered as transferable skills in order to meet minimum requirements of the position and will be considered by the Corporate Director of Human Resources. Associates or 2 years post-secondary education in social service or related discipline (4 years of directly related experience may substitute for education requirements) 2 years of supervisory experience Excellent verbal and written communication skills Knowledge of the DES/DDD Residential Program Monitoring rules Experience with writing and implementing objectives and skill plans per ISP guidelines Proven ability to lift 60 lbs 15% of the time Obtain State of Arizona Level One Fingerprint Clearance Card and pass drug screening Valid Arizona Driver License, proof of insurance and Arizona Motor Vehicle Division 39 month Motor Vehicle Report DESIRED QUALIFICATIONS Bachelor's in Social Work, Psychology, Sociology, Special Education or related field Additional years of directly related experience American Sign Language skills Completion of required CLP CDS modules
